EZ 2 Form for HUD Projects
Who is it for?
Entities in Washington that regularly receive and/or distribute federal funds from the U.S. Department of Housing and Urban Development (HUD). HUD refers to them as Responsible Entities for the purposes of Section 106 historic preservation review.
What is it?
An online form for submitting photos of buildings to DAHP so that we can help you determine if they are historic.
How do I start?
Step 1: Follow the WISAARD REGISTRATION INSTRUCTIONS to sign up for a Secure Access Washington Account and a WISAARD Account
Step 2: Follow the WISAARD SIGN IN INSTRUCTIONS to log in to WISAARD
Step 3: WATCH THIS VIDEO TUTORIAL to learn how to use the EZ2 Form and what to expect after you have submitted the form.
Step 4: Your form will typically be reviewed within a week. The video tutorial explains how to find the result of the review. There are two possible outcomes of the review and these are the terms you will see used in Wisaard: Determined Eligible or Determined Not Eligible. The terms refer to eligibility for the National Register of Historic Places.
1. Determined Eligible means the property is considered historic. We will need more information about the project. Please fill out an EZ Project Review Form.
2. Determined Not Eligible means the building is not historic and no further communication with DAHP is needed.
3. The property is already listed on the National Register and is considered historic. We will need more information about the project. Please fill out an EZ Project Review Form.
A few helpful notes:
1. Each form is for one building and one address. If you have more than one building, you will need to submit one form for each one.
2. If a building has been reviewed in the last ten years, you do not need to submit it again.
3. If the form is missing an address or a photo, we will return it so you can add that information. Once you add the information and resubmit, please also email us at 106@dahp.wa.gov and tell us the project number so we can expedite your resubmitted form.
4. For mobile homes: Please make sure the mapped dot is on the correct unit (not just the correct address) and that you attach a good photo of it (there is typically no Google street view for us reference if photos are inadequate).
