EZ / Project Review Form
Here in Washington State, in order to streamline the review process, we have developed an EZ/Project Review Form. The form should only be completed by project proponents that are not trained in cultural resources regulations and review requirements such as members of the public and community service organizations. The form is only for reviews in accordance with Governor's Executive Order 21-02 (formerly 05-05), and in special cases of Section 106 (HUD) reviews (note there are two slightly different forms that should be used for each). The EZ / PROJECT REVIEW FORM IS NOT PART OF THE SEPA PROCESS, PLEASE DO NOT SEND THE FORM FOR SEPA REVIEWS. We will review the SEPA checklist when it is provided to us by the local government. The goal of the EZ/Project Review Form is to determine if there are any National Register eligible properties that may be affected by the proposed project or if there is any potential to affect archaeological sites. To make these determinations we need a brief description of the project, a map of the area, and images of any buildings over 45 years old on the site. Please use the form (below) to submit the information. To complete an EZ/Project Review Sheet you do not need to sign up for Wisaard. Please allow up to 30 days for reviews of EZ/ Project Review form.
When do I fill out an EZ/Project Review Form?
TIP: Before filling out the EZ/Project Form, SAVE the file TO YOUR COMPUTER and fill it out in Acrobat Reader. This will enable you to add images and map to the form where needed.
1. Does your project involve any ground disturbance (fence posts, new foundation, grading etc)?
YES - Please complete the EZ/Project Review Form and email it to the appropriate address on the form. We will respond to your EZ/Project Review Form submission with a letter sent via email when we have completed our review. Our response will indicate if any further consultation is required.
NO - Proceed to Question 2.
2. Does your project involve demolition, maintenance, additions, upgrades, or rehabilitation of a building or structure that is 45 years or older?
YES - Please complete an EZ/Project Review Form and 1) verify in Wisaard that an Historic Property Inventory Form (HPIF) has alredy been completed and contains at least 1 image, the adress and a built date of the resource, or 2) fill out an HPIF using Wisaard to help us decide if the building may be historic (i.e. eligible for the National Register of Historic Places). In many cases staff can help you fill out the form. Please see this page for information about signing up for Wisaard and for a tutorial.
NO - You do not need to complete an EZ/Project Review Form If you have also answered NO to Question 1, no consultation with DAHP is required.
3. Is the building already listed on the National Register of HIstoric Places, or, was the building Determined Eligible (eligible for the National Register of HIstoric Places)?
YES - Just complete the EZ/Project Review Form and email it to the appropriate address on the form. We will respond to your submission by email.
NO - Consultation with DAHP is complete.
