EZ Project Review Forms 


State and federal agencies must consult with DAHP when they are spending or granting funds. Sometimes this process is done by professional cultural resources staff within the agencies and grantees are not even aware of it. But, in some cases, grantees are asked by agencies to consult with DAHP directly. We created the EZ form to make the process easier for agencies and grantees without cultural resources experience. 

Which form should I use? 

  • If your project/grant is funded by a state agency, such as the Department of Commerce, fill out the 21-02 EZ Form and send it to 2102@dahp.wa.gov. Download the form before filling it out.

  • If your project is funded by HUD or other federal agency, fill out the Section 106 EZ form and send it to 106@dahp.wa.gov. Download the form before filling it out.

Then what?

You should receive an email reply from us within 30 days. If you do not, please check back in. The email you receive back from us will advise on next steps, if any. If there are no further steps to take, you will receive a letter (digital, pdf) or a signed copy of the form via email. Either one will work to give to the agency funding your grant. 

Frequently Asked Questions about EZ Project Review 

Why do I have to do this?
  • You are a state agency spending capital funds OR you are the recipient of a state grant. The requirement was created by Governor's Executive Order 21-02.
  • You are receiving or granting federal funds. This federal requirement was created by Section 106 of the National Historic Preservation Act. 
I have multiple buildings or multiple properties for my grant. What do I do? Fill out Sections 1 & 2 the form the best you can. You can put additional addresses in Section 6 or attach a separate document with a list. Please also make sure to submit maps and photos of each property and each building. If they are adjacent, you may submit a single map.
How do I find out if a building in my project area is 45 years or older? If you do not know the age of the building(s), you can search the county assessor website to get an approximate date. To do this, search the name of the county followed by “real property search” in your web browser and follow their instructions. 
Do I also need to do the EZ2 in WISAARD? No. If you have been directed to this form, you do not need a WISAARD account and do not need to complete an EZ2 form. 
What is ground disturbance?  Ground disturbance is any digging in the ground, reglardless of where or for what reason. Check this box if there is any digging involved in your project.
How do I figure out which agency is in charge of administering my grant? If you received a letter informing you of your grant award, it may list what agency is in charge of your grant or a contact person you could ask. 
What does direct appropriation mean? Some projects are funded by direct appropriations, which means the legislature has put a line item in the budget just for that project. Even though the funding is not part of a particular grant program, the funding will still be administered by a state agency, usually the Department of Commerce, which must follow the same process as any other grant.
My grant administrator told me I need to conduct tribal consultation. Does DAHP do that? No. DAHP cannot consult with the tribes on another agency's behalf. Tribal consultation should always be done by the state agency administering the grant to ensure a government-to-government process. DAHP has a map and a list of tribal contacts to help you figure out who to contact. Please see our Tribal Consultation page for contact information.
 Can I send multiple attachments? Yes, but please send them all in one email if possible. If you send your map as a separate attachment, it does not have to be a .jpg. All photos must be .jpgs.
Do I have to use a map from WISAARD for my EZ form? No. Maps may be in various formats as long as we can clearly see the boundaries of the project area.
What kind of photos should I submit? Please do your best submit a clear, recent photo of each building that is not blocked by trees, fences, vehicles, or other obstructions. Google street view and county assessor photos are usually not sufficient as they may not represent current conditions. Photos should be JPEG or PNG file format. 
How do I submit my EZ project form? Email the form and any attachments in a single email to the address listed on the form. Either 2102@dahp.wa.gov or 106@dahp.wa.gov.
How long will it take to hear back from DAHP about my EZ form? Typically within two weeks. If you do not hear back in 30 days, please email 2102@dahp.wa.gov requesting an update.
I am doing a project under local laws such as SEPA or Shoreline Management. Should I fill out this form? No. 
When in the process of applying for and receiving my grant do I need to fill out the form? It depends on the agency funding your grant. Usually, you will need to fill the EZ form out and wait for a response from DAHP before the agency will allow you start any part of the funded project. Sometimes you may be asked to fill it out before you apply for a grant. 
I have several project alternatives and we have not chosen the final one for our project. What should I do? You will need to submit the form with information about all the alternatives. DAHP must know about all alternatives to be able to provide a complete review.
My project has changed. Do I need to submit another EZ project form?

It depends on what the changes are. Please email us your original form and a brief description of the proposed changes and we will respond with next steps.
